Urinary flow is impaired because the urinary tract is surrounded by the prostate, which restricts the flow of urine when the prostate becomes enlarged. The underlying cause of prostate enlargement can be attributed to aging-related hormonal alterations. These hormonal shifts result in enhanced levels of two metabolites of testosterone that adversely affect prostate function estradiol (the predominant type of estrogen) and dihydrotestosterone (DHT). Specifically, these hormonal changes are 1) a decrease in testosterone production, 2) an increase in estradiol production, 3) a corresponding decrease in testosterone to estradiol ratios, and 4) an increase in DHT levels. Prostate cells are sensitive to estrogen's growth stimulatory effects and, thus, estrogen stimulates proliferation of the stromal cells in the prostate (in part by inflicting DNA damage to prostate cells). This cell proliferation, coupled with the prostate-thickening effects associated with DHT, results in unwanted prostate enlargement that is the cause of the numerous urinary discomforts being experienced by aging men.
